We aim to treat people the way in which we would want to be treated ourselves.

We are committed to the development and training of our staff to ensure our patients are given up to date evidence-based care.”


Vision Statement

 Eastfield Medical Centre looks after over 8000 thousand patients. We have a close team of Doctors, Nurses, Nurse Practitioners, HCAs and support staff working together to provide the best care we can in a safe and caring manner.

With our local population growing we will continue to seek more efficient ways of working without compromising on the quality of care we strive to deliver.

We will continue to embrace and explore community-led research, development and quality improvement.

We will continue working closely with Health Care professionals who work within the community. We will engage with upcoming community projects to ensure that our most vulnerable and complex patients have better continuity of care.

 

We Aim to: 

  •     Deliver high standards of holistic Medical Care using available resources
  •     Ensure that patients with chronic diseases are given specialist and tailored care which is made possible via our extensively trained Nurses and HCAs
  •     Continually look for ways to ensure continuity of care
  •     Treat all patients, carers and staff with dignity and respect
  •     Be courteous, approachable and understanding of our patients and each other
  •     Deliver a patient centred service through shared decision making and good communication
  •     Safeguard children and adults by adopting best practice and local guidelines and pathways
  •     Attend meetings and engage with other GP practices to ensure we are working as collaboratively and efficiently as we can
  •     Engage with the Commissioning of services where appropriate
  •     Encourage patients to join our Patient Participation Group
  •     Encourage patients to learn more about their condition through education and promote self-care where appropriate
  •     Maintain a clean, safe, welcoming environment
  •     Act with integrity and complete confidentiality
  •     Operate in the spirit of and in accordance with the legislation of diversity and equality
